인컴이 낮은 분들을 위한 프로그램 MLTSS
Medicaid 의 한 프로그램으로서 인컴이 낮은 분들을 위한 프로그램으로 아파서 널싱홈에 가야 하지만 본인의 집에 머물기 원하시는 노인 환자나 장애인을 위한 뉴저지 주정부 프로그램 입니다. 메디케어가 있으나 메디케이드가 없는 분으로 인컴이 정부 보조 프로그램의 기준에 맞는 분에 해당 됩니다.
- 버겐 카운티에 거주하는 65세 이상 혹은 18세 이상 장애인
- 영주권, 혹은 시민권자
- 만성질환으로 일상 생활이 힘드신 분
MLTSS 프로그램 인컴 조건
- 개인 인컴 월 $2,250 이하
- 개인 은행잔고 $2,000 이하
- 커플 은행잔고 $3,000 이하

A: 네, 나이 제한이 있습니다. 65세 이상 되셔야 합니다. 하지만 선천적 혹은 후천적 장애자의 경우는 18세 이상이면 됩니다.

A: 노인 환자로서 혼자서 생활 또는 거동이 힘드셔서 누구의 도움이 꼭 필요로 하는 분 또는 의료 보조기구인 지팡이, 워커, 윌체어에 의존 하셔야 하는 분들에 해당합니다. 또는 만성 질환이 있어 일상 생활이 힘드시고 의사의 진단을 받으신 분들에 해당 합니다.
Q: 주정부 연결 서비스에는 어떤것이 있나요?
A: 주정부 연결 서비스로는 주요하게
- 홈케어-간호사, 혹은 홈 에이드가 집으로 방문하여 환자를 돌보아 줍니다.
- 의료장비 보조 (지팡이, 워커, 윌체어)
- 식사배달(주중 하루 1번 혹은 일주일에 한번)
- 간병인(가족)을 위한 프로그램
- 의료 데이케어
- 처방약 할인 및 보험료 할인 프로그램 (PAAD, Medicare Saving Program)
- 소셜 어덜트 데이케어
- SSI, Food Stamp, Life line (Utility, Telephone, Gas Assistance)

A: 영주권자 혹은 시민권자여야 하고, 몸이 아픈분의 경우는 영주권 취득 후 꼭 5년이 되지 않아도 신청 가능 합니다.

What is Care Management?
The Care Management Program is a Consumer focused collaborative process which assesses individual’s needs and plans, implements, coordinates, monitors, and evaluates options and services, using formal and informal resources, to meet home bound frail older adults and their caregivers’ needs. This program is under the New Jersey ADRC system.
A care manager is someone who is a registered nurse, social worker and/or other health related professionals who is trained by the New Jersey ADRC system. A care manager is responsible for care planning, monitoring of care needs, service delivery, and assisting with various programs and services to help individuals to remain in the community.

Donations
There are no costs to the individuals, but the Program is dependent on voluntary contributions to help defray the cost of the service. All donations are voluntary. No one is denied service because of non-contribution.
